Summary: Missing attributes on some custom forms
Initial Comment:
I created a template for my own needs, and noticed some fields were missing.
I dug down the problem and found a fix; patch attached.
It seems the Attribute::ldaptype for the missing attributes was set to "optional". I guess it ain't right.
If you need more info, ask without hesitation.
