If you click the "Add" button in the InfoLog list, the preselected type is "note" and the preselected status is "done", regardless of what you set the default type to.
This seems to be beause the target URL of the button is
index.php?menuaction=infolog.infolog_ui.edit&type=note&action=&action_id=0&cat_id=
In contrast, when you use the "Add" link in the left menu, the target URL is
index.php?menuaction=infolog.infolog_ui.edit
which is more likely what the user expects.
