MONITOR spec from revision 26703 of ratbox ircd's SVN trunk
We should make an attempt at using MONITOR in WNO, which shifts the notify list
to the server and thus avoids polling (however, since the server-side list is
of finite size, a hybrid approach that handles a possible overflow via ISON
would be required).
