If you've set the volume of your system to 60% the kscd stream in pavucontrol will show up as 60%. This is known as "flat volumes", but that's not really important. The important factor is that pulse doesn't have a "default" volume of 60%. It's whatever you've chosen. As for the 40% bit in kscd, I have no idea about that. If it has it's own volume system and decides to attenuate the sound to that level before playing it, then this is not a pulseaudio problem. I can't really see that this is anything other than a kscd defaults issue (i've not tried kscd personally, so I'm just trusting you that it's 40% by default).
