Support GENERATED ALWAYS AS in index definitions

Since 2.0, we allow COMPUTED BY in an index definition.
Since 2.1, we support the SQL-2003 compliant GENERATED ALWAYS AS syntax.

Therefore, we should also allow GENERATED ALWAYS AS in an index definition
(currently this gives an error - tested in 2.1.1)
