I have been looking at Cmap and it looks more complex than I thought:

it could be an "as is" map, each 16 bits char value {0, 1, 2, ... 65535} will be
represented by its same glyph "position". It might be used
by latin languages and as a template to create Cmaps that offset some
portions, as your code does, but don't trust me.
This is an example of CJKV and Chinese + Japanese font support.

I think most probably a Cmap that suites C+T already exists,
it should be among the 31 following Acrobat Cmaps:
Adobe-GB1-xxx
GBxxx
UniGBxxx
UCS2-GBxxx
This is very hard, as once encountered it must be ported to TCPDF;
for this reason I've been lookin for a TCPDF map, but no luck.
Maybe it's time to contact TCPDF and Adobe.
