If you're looking for the reading of content under the mouse pointer in Windows, you'll also find it in several of the GUI based screen reading applications for persons with reading difficulties. I would suppose you'll find it in TextHelp's Read&Write software. Here in Scandinavia we have ViTal and WordRead Plus etc. 

In the Internet environment in particular you'll find BrouseAloud http://www.browsealoud.com

 

Unfortunately not any FOSS alternatives around in this flavour yet as far as I know.t

 

The problem with the content reading may be that it may make you really tired by talking all the time. You need good functions for making it shut up. Ideally also to just seak on demand.

Yes, Orca has a setting that reads out what is under the mouse, but also
provides feedback with keyboard navigation which will help your friend ease
their way into using a full screen-reader. It is better to learn how to use a
screen-reader while you can still see. drbongo
