ESMTP server needs to figure out the receiving mail server through DNS MX/A records. A possible gotcha here is you might need to forward requests for internal addresses/local domains to your main DNS server if needed - otherwise you may find internal addresses it needs to resolve either returning one of your external IPs or not resolving at all.
