Examples of the form:

set sendmail="mail.remote smtp://hostname[:port]"

should be changed to read

set sendmail="mail.remote --mailer smtp://hostname[:port]"

The latest version of mail.remote built from mailtuils does not
recognized the smtp option without the --mailer switch.
