ERR_ACCESS_DENIED despite ACLChecklist...answer=1

I'm getting 403 Forbidden responses from my accelerator-proxy; I
turned up the ACL debug level to see what was going on, but the log
shows that it was accepted by "ACL: ACLChecklist::checkCallback:
0xb85ddcd8 answer1", so I don't understand why it's getting 403'd. Is
there somewhere else I should be looking?

There are multiple checklists which may or may not pass.
Was that the checklist for http_access?

* check that your reverse-proxy configuration http_access lines are
above any other http_access lines for regular proxy access.
