Example of a form to add an embedded youtube movie in a page?

Some old (i.e. copyright expired) movies on youtube are split into
several pieces, and I was thinking it would be really nice to build a
SMW for managing these kinds of 'playlists'.

I read about the MetavidWiki Extension:

But Metavid seems a bit more than I want.

Basically I want to make a simple, form-based list editor combined
with something like EmbedVideo[1]. Are there any good examples out
there? I didn't find one here [2].
