W3C is a web standards body, and this list is just for discussing those standards. So unfortunately we can't help you here.

If you're new to HTML forms and need help, you should probably check out a site such as w3schools.com

Or if you have a specific question or problem, try asking on a website such as stackoverflow.com - I'm sure they would be more than willing to help.
