Proxy constant name change

Dave,

I've added "PTP_" to the beginning of all constants in the proxy C  
code to avoid namespace issues. I think I've updated everything in the  
LL and PE proxy, Can you check to see if I've missed anything?

There were a couple errors in the LL proxy which I fixed. The PE proxy was ok.

The codes in debug subsystem (such as src/utils/error.c) needs to be updated with the new proxy constant names.

Have you updated the C plugins (org.eclipse.ptp.debug.core and org.eclipse.ptp.proxy) from CVS? These are building cleanly for me.
