You confirmed in another message that this is about HTML Help (a variant of
chunked output).
Try setting the chunk.first.sections parameter to 1. This will generate a
separate HTML file for the first section in each chapter. See
http://docbook.sourceforge.net/release/xsl/current/doc/html/chunk.first.sections.html
