In ArcPad (by itself), you can set up a "quick form" that will at least address populating attributes in a shapefile as you add records.

ArcPad does not include App Builder, but the reverse is true (App
Builder does come with 1 copy of ArcPad).
Also, ArcPad does come with a basic form building tool, from what I
understand, so you don*t necessarily need App Builder if you*re
doing basic apps. Can anyone verify that this is true?

these are 2 different software packages. I asked if Application
Builder came with ArcPad, it does not.
ArcPad is currently $495, and Application Builder is $1500.
