I experienced these problems several times.

my approach is as follows:

- set your Yaw between the shots manually in Image Parameters ( you  
know the angle more or less)

- don't use the project assistant for placing the CPs

- set CP either  manually or " half automatically"  in the CP  TAB  /  
Menu Control Points Generate CP for  a pair of pictures at a time.

that normally resolved the problem.
