I think you are trying to change a bit to much to quickly; the
"There's no need ..." paragraph contains useful information that was
removed.  Some small changes that I don't understand, why was help
files changed to media files?  And that you are trying to make the
description field mandatory is my biggest issue.

I'd always use real examples instead of `templateish' snippets; they
add the additional information of where to look for ChangeLog entries
in the real world.

I'll send a suggestion tomorrow...
