Yes, works in gpg4win 2.0.1rc1 and I could just complie it
and run it for pinentry-0.7.6 on lenny.

This part works.

the gpg-agent "can't connect server", "can't connect to the PIN entry
module" errors are the odd ones here it seems.

Any other advice on finding where the gpg-agent+pinentry-qt4 combo goes
wrong?

You can try setting GPGMEDEBUG.
Make sure the gpg-agent.conf has the right debugging options and pinentry
configuration.
