Strange issue

MacBook Pro running 10.4.11
2.2 GHz Intel Core 2 Duo
4 GB SDRAM

After a recent Safari Beta 4.0 update and restart, I received the following
window that will not close:
 
Failed to connect to the device.
-Check the I/F cable connection between the device and your computer.
-Please true OFF the device once, and then turn ON the device again.
They try again
            [MTW005]

This included a small Icon with a picture of a camera.

 
I have had no camera, or any other device connected, except for a USB mouse.
 
Since then, I have disconnected the mouse, run Disk Utility and Applejack to
no avail. Monitored the ActivityMonitor.app but nothing was obvious, but I
am just an amateur.

I did find this, repeated several time, in the system log:
AppleUSBUHCI[0x6657000]::Found a transaction past the completion deadline on
bus 26, timing out!
May 23 10:13:21 XXXXXXX-XXXX-MacPro kernel[0]: USBF:

 
Before I dive further into this issue, and make matters worse, I was
wondering if anyone had any suggestions.

Google for MTW005. Looks like Image Capture takes your mouse for a
camera. Change its preferences so that it doesn't open anything
automatically.

Thank you. I found two versions of the Image Capture.app, an older 2.02
version and a newer 3.03 version. After trashing the older version and
running Disk Utility again, the problem is now gone.
