Translating into Chinese

Hi,

I am at Intel Research and plan to conduct an online survey in Chinese (mandarin). I am looking for a person or a translation service that could translate our English survey into Chinese and also create the web survey with the Chinese characters - either using Survey Monkey or a Chinese online survey service. Any recommendations are very much appreciated :).
