The confluence captcha settings look good (you only need to do the
captcha if you are not logged in or are creating an account), but the
captcha image currently doesn't show up... which is a little too
aggressive.
We'll need a friendly volunteer from Refractions to help configure the
server. The confluence logs might offer a hint, but my shot in the
dark is:

Administration > Configuration > Spam Prevention
