amavisnewsql whitelisting causing sender's address to be the digest_from address

Just finished upgrading the mail environment.  The QA team noticed that something terrible occured when they did the following things:

1) They added an email address - we'll call it mytest@domain.com - to their whitelist. (amavisnewsql)
2) They sent an email to mytest@domain.com.
3) When the email arrived at mytest@domain.com it was "from" the email configured as the "digest_from" in the amavisnewsql config file.

It is beyond me how this could even come close to being possible!

Any help would be greatly appreciated.  (I will provide any software versions, plugins installed, etc)

Can't help you with that plugin, but I'd recommend the Server Settings
plugin as a more up to date replacement for it.
