books with index produce a blank PDF bookmark
When we add an index to a book and then publish a PDF (using the
unmodified default PDF publication type that comes with Daisy), the
resulting PDF is generated with a blank bookmark where the reference to
the Index should be. It is a clickable bookmark, and it does move you
to the index if you click it, but it's blank. Just to be sure, we tried
this with a very small document created from scratch, and the problem
still occurred.

doesn't get forgotten. If the issue is urgent, see [0] for a tip that helps you disect and analyse the publication process.
