dWorldStep without alloca

At last I have found some time to find and fix the bug in my redesign of dWorldStep() to get rid of alloca() and now I can present the community the raw version of the changes.
It is still necessary to remove some dead code and unused preprocessor defines. Also, I'm going to do the final code review yet. However the changes attached are fully functional and complete.
