deleting files attached to content

On a node/page/story I can upload and attach a file.  It appears on
the page, and in the file folder on the system.  When I flag the file
for deletion, it is removed from the page, but it remains on the
server.  I would think it was a permission issue, but it gets created
on the sever to begin with.

Any suggestions?
