edits to Chapter 5

Attached is the patch file for details-institutions.docbook.  I left  
the main two paragraphs, but I wonder if it would be better to break  
them into separate paragraphs (or perhaps a bullet list?) for each menu  
option.  Also, for the Account options - I'm not sure if it would be  
better to include a description for all the available options, or list  
fewer details and just point to the Accounts chapter?
