minor change to the "Lab Req 2007" formI'm sending this to both the devel and users list because it may affect users who currently use the "Lab Req 2007" form.
I've made a minor change to the form. Previously on the form, on the right had side, there was a line with the following : "Patients Name" , "Health Number", "Version", "Province" (Where province was the patients address province) I was asked to change province to the health card province / heath card type. I've also changed the label of the field so it's more clear. Re: minor change to the "Lab Req 2007" formno I am not, I don't know where the "paper" form is and haven't ever seen it.

Re: minor change to the "Lab Req 2007" formEffective January 1, 2009, the Ontario Ministry of Health and
Long-term Care (MOHLTC) changed the way Prostate Specific Antigen (PSA) tests is covered under the Ontario Health Insurance Plan (OHIP). As of this date symptomatic men could have the test paid for by OHIP. To accommodate this, as of January 1, 2009 the OHIP form changed. If you have OSCAR installed or updated after that date then the "form" corresponding to the link labeled Lab Req 2007 (and its formlabreq07.jsp) will have been updated to produce the new spec form.
