Effective January 1, 2009, the Ontario Ministry of Health and
Long-term Care (MOHLTC) changed the way Prostate Specific Antigen
(PSA) tests is covered under the Ontario Health Insurance Plan (OHIP).
As of this date symptomatic men could have the test paid for by OHIP.
To accommodate this, as of January 1, 2009 the OHIP form changed. If
you have OSCAR installed or updated after that date then the "form"
corresponding to the link labeled Lab Req 2007 (and its
formlabreq07.jsp) will have been updated to produce the new spec form.
If you are not sure what version you are using, you are up to date if
your form has the box on the right that has the following:
Prostate Specific Antigen (PSA)
Total PSA Free PSA
Specify one below:
Meets OHIP eligibility criteria – Insured test
Screening purposes – Uninsured test
