This sounds very familiar...

This may be related to a new feature that Jan added on October 13,
adding the DisableRTPQueueing flag, though the older default behavior
(DisableRTPQueueing=0) should have remained after the fix.

We _were_ having a problem in specific call situations where RTP
packets were errantly being sent back to the originator, and it was
breaking audio in one direction.

Jan added this flag to allow changing the default behavior so that
such RTP packets would be dropped outright and not forwarded in that
case.

Try enabling DisableRTPQueueing=1 in your [Proxy] section and see if
it helps at all.

We have been running substantial call loads with this flag enabled,
and it has been working well for us so far. It is a new flag though,
so YMMV. It might not fix anything for you at all.
